Screening Day & HIPPA
The Health Insurance Portability and Accountability Act of 1996 (HIPPA) was created in part, to address the security and privacy of health data. This Act ensures that personal health information (PHI) shared in a health assessment campaign, for example, cannot be used "against the individual" for insurance coverage or employment consideration. Who is conducting the screening?
UnitedHealthcare, and The Wellness Group (Dr. Jill Waggoner) have partnered with the City of Dallas to provide an Employee Wellness Program for Employees, Retirees, and their dependents.

What steps are taken to ensure me of my privacy?
At the designated screening locations the chain of custody of the information is as follows:

For Your Privacy and Our HIPPA Compliance

Weight, Height, Body Measurements

Measurements are taken and recorded on a form that is handed directly to you.

No copy is retained.

Blood Work

The contracted lab company draws the blood.
The results are sent to Dr. Jill Waggoner for review.
After reviewing Dr. Waggoner mails the copy of the results to the participant.

The lab results are only seen by the lab and the physician.

After reviewing the physician does not keep a copy as no medical record is started.

We suggest you send a copy to your primary care physician for your medical record if you so choose.

When will I get my results and who will have them?
You will receive your results in within 2-3 weeks after being screened. The chain of custody for your lab results is the lab company and Dr. Waggoner. No copies of your results will be retained by the physician. Once the physician mails the copy of your results you are the only one who has a copy.
